Information At Your Fingertips
If you are a new patient to our clinic, please arrive at least five minutes before your consultation or fill out a new patient form here and bring the following to your consultation:
- Photo ID
- A valid Medicare card
- Workcover patients will need to inform their employer and bring their employer’s contact details for verification to arrange payment.
Fees
Billing Policy
We are a mixed billing practice. Most consultations are time based and will incur private fees for patients who are not eligible under the bulk billing criteria.
Fees are payable at the time of consultation by cash, credit card or eftpos.
We are still able to bulk bill eligible patient groups for most of the consultation types:
Bulk Billing is available for:
- Children 15 years and under
- DVA Gold Card Holders
- Pensioner concession card holders 65 years and over
Services excluded from bulk billing include:
- WorkCover
- RHCA Medicare cardholders
- Procedures
- Cervical screenings
- Pre-employment medical assessments
- Medical/other reports
- Skin cancer checks
- Drive Medical Assessment
(Taxi/Uber/Diabetic/Racing) - Cosmetic Treatment’s
- Urine Drug screening
- Allied Health services
- Transfer of History

Home Visits
Home Visits can be arranged in certain circumstances with a pre-booked arrangement with the doctor on call. Please contact the receptionist to discuss your needs. Remember; always call the ambulance service on 000 if you are experiencing severe pain or illness at any time.
Contacting Your Health Professional
Phone calls to your doctor will be put through initially to the nurse if available or the nurse will return your call as soon as possible. The nurse will then discuss any concerns with your doctor andnget him/her to phone you back as required.
If your doctor has asked you to phone, please advise reception and you will be put through or a message will be taken for the Doctor to return your call as soon as possible.
Booking Appointments
Appointments may be booked via online (Hotdoc) or by calling: (02) 4704 3710 in person Monday to Friday 09:00 am – 05:00 pm. Regular appointment times are 10 minutes, if you feel you need longer, please discuss with the receptionist at time of booking.
New Patients Accepted
Appointments for health assessments, complex medical problems or removal of skin lesions and wound management, etc. all require extra time. Whilst the Doctors aim to keep patients waiting times as short as possible, delays may occur.
Prescriptions
Check your medications regularly and ensure you have sufficient supply until your next appointment. An appointment is required for all repeat prescriptions.
After Hour Services
Please call 13SICK (137425).
For emergencies we recommend contacting the local hospital. If you need medical advice, you can contact Healthdirect on 1800 022 222.

Confidentiality-Privacy and Health Information
Your medical record is a confidential document. It is always a policy of this Practice to maintain security of personal health information. Information is only accessed by authorised members of staff for the purposes of improving or enhancing your health care.
Reminder System
Reminder System Silverdale Doctors is committed to preventative health care and has a recall and reminder system in place for routine screening and care plans. You may get a text message, phone call or letter from time to time for the appointment.
Results
A follow up appointment is required to receive results. Telephone advice will only be made if authorised by your doctor. Silverdale Doctors will attempt to contact you regarding any urgent results.
Compliments and Complaints
If at any time you have a concern, comment, or suggestion, please do not hesitate to fill in a Compliment, Comment, or Complaint form available in the Silverdale Doctors waiting room. Alternatively, you can contact the Health Care Complaints Commission by calling 1300 582 113 between 9:30 am and 3:00 pm, Monday to Friday, or by mail at: Level 12, 323 Castlereagh Street (Corner of Hay Street), Sydney NSW 2000.

What is MyMedicare?
The Australian Government has introduced a new voluntary patient registration model called MyMedicare to drive improvements in primary health care for all Australians and deliver new funding packages to primary care providers. MyMedicare will strengthen the relationship between patients and their health care teams by allowing patients to register with their regular general practice and select their preferred GP and providers within the practice. This registration aims to strengthen the relationship between you, your GP, and the team at Silverdale Family Doctors. What are the benefits of MyMedicare for our patients?
The key benefits to you include better access, better continuity of care, and better planning. Registration in MyMedicare is not compulsory, however it is recommended to improve continuity of care for patients. You can also change your mind and leave the program at any time.
